Highpoint Homecare offers convenient, cost-effective and reliable care for patients who have healthcare needs at home, often after they are discharged from the hospital. Our trusted care and constant attention to the needs of our patients not only benefits them, but provides much needed peace of mind for their families.

Admission Criteria

To qualify for home health several criteria must be met:

- - Patient must be homebound
  - Patient must be under the care of a physician
  - The care must be provided in the patient's place of residence (home, assisted living facility, group home or family care home)
  - The services must be skilled and needed on an intermittent basis

Most referrals come from a hospital, physician, rehabilitation or skilled nursing facility.

Highpoint Homecare can provide a service or combination of services in your home. Working with your physician, our qualified staff will plan, coordinate and provide care tailored to your needs. Our services include:

Service Area

Highpoint Homecare has offices in Gallatin and Carthage. Our office hours are Monday through Friday from 8:00 am to 4:30 pm, except during company holidays.

Services are provided in Sumner, Davidson, Robertson, Wilson, Trousdale, Macon, Smith, DeKalb, Putnam and Jackson counties.

HighPoint Homecare service area

We have a nurse on call 24 hours a day, 7 days a week to ensure patients receive necessary home care services.

We offer a wide variety of quality services, ensuring that patient care needs are met in the privacy of their own homes. Additionally, Highpoint Homecare provides professional health care in many area long-term care facilities.
